Physicians may refer by phone, mail or fax. Please include: date of referral, client name, date of birth, reason for referral, work and home phone number and address with postal code.
Parents and clients may self refer by phone. Please leave a brief message with both work and home phone numbers.
A referral is always required from physicians and public agencies. For clients
it is customary to get a referral but it is not required in order for us to
provide you with services. You will require your physician’s referral
however in order to access insurance funds. Your insurance company will request
the original referral and we take a photocopy of it for your file.
